The Cardiac Rehab Liaison serves to consult with all patients about Cardiac Rehab per provider orders. The Liaison duties include: strengthen physician-patient and staff relationships, educating about program expectation to patients and families; coordinating appointments; promote clinic to a variety of provider populations; developing and implementing strategies to enhance communications with physicians and other health care providers; increase volume for the clinic; and ensuring customer satisfaction.
Primary Duties:
- Develops, maintains and strengthens positive relationships with referring provider and patients to optimize Cardiac Rehab usage.
- Consults with each patient regarding Cardiac Rehab per provider orders.
- Schedules patient’s initial evaluation at Cardiac Rehab clinic.
- Documents interaction with every patient.
- Enters patient information into electronic medical record if needed.
- Provides tours of Strong Hearts Rehabilitation Center to new patients as needed.
- Other duties as assigned by immediate supervisor based off demonstrated competency.
- Directs communication and complaint resolution processes to ensure high service quality and provider satisfaction.
- Proactively solicits feedback from patient and provider.
- Fosters accessibility of Cardiac Rehab Director for referring provider’s questions and concerns.
- Relays provider concerns immediately and ensures timely action and follow-up.
- Serves as liaison between medical staff and referring providers regarding requests and referred patients to ensure continuum of communication and care.
- Enters orders into EMR system.
- Assists team with direct patient care as needed.
- Adheres to strict confidentiality both internally and externally.
- Performs all duties in accordance with Arkansas Heart Hospital mission, vision and values.
Qualification/Specifications:
- Education: High School diploma or GED. Associates degree in business, marketing, management, communications, nursing, healthcare administration or related field required. Bachelor’s degree preferred.
- Licensure/Certification: BLS certification by 30 days of employment required. Certification is required to be updated by recommended renewal date on documentation.
- Experience: Minimum of 1 year experience in healthcare sales and/or marketing required. Will consider professional-level healthcare experience, including ongoing coordination/communication with physicians, in lieu of sales and marketing.
