Position Summary
Responsible for the planning and coordination of wound care for AHH’s hospitalized patients. Manages day-to-day patient care processes including but not limited to: working with management and nursing staff to ensure the hospital is maintaining safe clinical practice; meeting TJC and CMS guidelines; coordinating with case management and appropriate outside agencies; collaborating with other departments as appropriate. Accurately manages expenses while providing high quality care.
Work Schedule
Forty hours per week, Monday through Friday with occasional weekend hours when necessary. Hours and days will vary to accommodate the needs of the department and hospital.
Primary Duties
- Provides leadership in the formulation of objectives, goals and policies for the wound care in the hospital.
- Performs patient assessment in response to Wound Care consult, and as high-risk patients are identified during daily surveillance, collaborates on multi-disciplinary team to facilitate evidence-based wound care management.
- In coordination with clinical services management, identifies the educational needs of all staff relative to advances in wound care treatments as well as changes in TJC and CMS requirements and ensures staff is trained accordingly.
- Assumes responsibility for appropriate resources, including but not limited to, specialty wound care supplies and equipment as required.
- Communicates post-discharge care needs to the multi-disciplinary team, such as Case Managers, Nurse, Providers, to effect for a safe and effective transition of care.
- Serves as a role model in appearance and patient care focus.
- Utilizes effective leadership communication techniques with physicians, staff, outside vendors, patients and ancillary departments.
- Adheres to mission, vision and values of the hospital and follows objectives set by AHH administration.
Qualifications/Specifications
- Education: Completion of accredited RN program. BSN preferred.
- Licensure/Certification: Registered Nurse
- Experience: Minimum one year wound care experience preferred.
